I am in most Utah Insurance Networks.

What this means is that I can still see clients with out of network fees with their insurance. You can check with your insurance provider if my services would be covered. You can tell them you would like to see Amy Davis, an LCPC (Idaho)/CMHC (Utah), who is out of network and see if they have out of network rates.

I also provide private pay sessions and those fees are labeled below.

Professional Fees: $140/55 min session.

I am now In-Network with SelectHealth as well as Blue Cross and Blue Shield, Optum/United Behavioral Health in Utah and accept benefits for services through them.

Payment is due in full at time of service. My electronic health records service allows you to pay via credit card through the portal, and/or print out your own super bills for services.
